What is a Mental Health Treatment Preference Declaration?

A mental health treatment preference declaration allows you to choose whether you wish to receive electroconvulsive treatment (ECT) or psychotropic medicine when you have a mental illness and are unable to make these decisions for yourself. This can be a written statement or you can have an attorney-in-fact make the decisions for you. Two people outside from health care professionals and family members must witness the declaration signing and this declaration will expire three years afterward.
